Start off with a large blank canvas - I used 1800x1000 in my example. Create a new layer to work with - we'll be rendering some ice here.
Set your background palette colour to a light blue (e.g. A7CCFF) and your foreground to white. Render some clouds using Filter » Render » Clouds.
Apply two filters to the layer: - Filter » Distort » Ocean Ripple
Set the ripple magnitude to maximum - Filter » Sketch » Bas Relief
With default values

Create a new layer, and fill it white. Reduce the opacity to around 35% to lighten the ice we rendered - we'll use this soon. Duplicate the ice layer and bring it to the top, then flip it vertically (Image » Transform » Flip Vertical).
Use the text tool to type your text. Do this near the top of the image, where the ice should be darker. Bring the ice to the top again and make it a clipping layer based on the text below
Things should be taking shape now - check the image and layers on the left against what you have so far.

Select the text layer and apply a liquify filter (Filter » Liquify). Use a small, hard brush and drag downwards to create "icicles" hanging from the text. Fast, vertical strokes work best!
Next apply a slight gaussian blur (Filter » Blur » Gaussian Blur) of a couple of pixels
Finally, add an outer glow (Layer » Layer Style » Outer Glow) of about 8px size and 20% spread - white. This gives a fairly dense glow to add some definition to the text.
